Subcontractor Prequalification

Please Note: The following documents are required to complete the prequalification process and must be uploaded with your submission. If you do not have all required documents readily available, please wait to begin the prequalification form until you have gathered them. The form cannot be saved, and incomplete submissions will require you to restart the process.
Required Documents:
- Bonding Capacity Letter – Issued by your surety company and stating your:
- Single project bonding limit
- Aggregate bonding capacity
- Current available bonding balance
- Experience Modification Rate (EMR) Letter – Issued by your workers’ compensation insurance carrier or agent showing your EMR for the most recent three (3) years, at a minimum.
- Union Fringe Benefit Verification Letter (Union Contractors Only) – A current letter from your union benefit fund administrator confirming that all fringe benefit contributions are paid in full and your account is in good standing.
- Certificate of Insurance (COI) – A current certificate clearly identifying your insurance coverages and policy limits.
- OSHA 300 Log – Your current OSHA 300 Log, completed through the most recent reporting period.
Providing complete and accurate documentation will help expedite the review process. Incomplete submissions or missing documentation may delay qualification or result in your firm being deemed ineligible for award consideration until all required information has been received and reviewed.
